Transfer of Records
Up to date details about my new arrangements can be obtained by following my Twitter (Ray @raysilins_ray). Advertisements will also be placed in local papers soon. If you wish, you can now obtain a copy on CD of your medical record by signing a consent to release medical records form and then forwarding it (in person or by post) to your current practice.
The consent form is available by clicking on this link, or you can pick one up from reception at Hunts Avenue. If you have any problems with any of this, please call Becky, Practice Manager at Hunts Avenue on 9804 8233 to assist you. You are entitled to take this medical record yourself and then give it to the doctor/practice of your choice. You may also ask that the record is mailed to the practice of your choice. You may choose that the record is mailed to Dr Ray Silins, Hunts Avenue Family Practice, 1a Hunts Avenue, Eastwood 2122.
The original medical record at time of your request will remain with that practice.
If necessary, I am able to see you prior to your medical record transfer being organised.
